Medical Report
Each prospective student is required to submit a Medical Report.
The Medical Report must be completed and signed by BOTH the parent AND the physician.
If you are unable to secure an appointment before turnig in the application, please complete your portion of the Medical Report, make a copy of the Medical Report and submit the copy with the application.
Take the original to your child's physician and ask the physician to complete the form at your child doctor's appointment.
