MRCA Sports Information

Each student planning to tryout for any athletic team at Mitchell Road Christian Academy must have the following forms completed and turned in to the Athletics Office before tryouts:

• Pre-Participation Physical Evaluation Form: This form must be completed after June 1st in order to be valid for the following school year unless there has been a change in the student's health. 

• Release of Liability Form: This form is valid for the entire school year unless there has been a change in contact numbers and/or insurance information.

• 2023-2024 Athletic Handbook: The Acknowledgement Form located in the Athletic Handbook must be signed by both the student and parent. This form must be completed each year.

• Student/Parent Concussion Awareness Form: This form should be signed by both the student and a parent.
